Project plan: Who is responsible for what?

We all know that the project plan is one big document, and it contains a lot of information from many sources. I would like to know who is responsible for what in the project plan. Is the project manager responsible for each and every part of the project plan, or is the responsibility shared with others?

Much of this depends on the size of the project and the project team.  Small project, yes, the project manager would be responsible (and possibly accountable) for facilitating development of the entire plan.  Larger projects, the project manager will coordinate project management assistants and team leads to put together a comprehensive plan.
